Innovation in practice: Translating evidence into better outcomes for patients

About this project

I wrote this booklet for the Prevention and Care division of the British Heart Foundation. Aimed at stakeholders and healthcare professionals, it showcases the department’s initiatives and the impact that these have had on cardiovascular health services across the country. I also edited the booklet on layout, using Indesign.

Targeting inequalities

People in the poorest areas of the UK die, on average, several years earlier than those in the richest – and heart disease is a major cause. Here’s how our Hearty Lives initiatives are helping those most in need.

To date, we’ve invested in 38 Hearty Lives projects in areas of social deprivation and health inequality across England, Scotland, Northern Ireland and Wales. Through them, we are creating effective and strong working partnerships with local authorities and health services to develop preventative health programmes and new models of care. This is a selection of projects that have helped transform the lives of more than 128,000 people.
